Vaccination Status Qualification

Hi all,


I'm wondering how others are handling vaccination status questions, or if you need to.


Our local heath department is requiring employees and volunteers to answer the below question:


Please select the statement below that accurately describes your vaccination status:

  • I am fully vaccinated. My last dose of the __________ (Pfizer, Moderna, Johnson & Johnson

/Janssen) vaccine was on ___________ (Date)

  • I have not been vaccinated
  • I decline to answer whether I have been vaccinated

I am trying to figure out the best way to make this work within Galaxy. It seems like a waiver within Qualifications would be the way to go.

My concern with this is how we would export any of that data at scale in a meaningful way. 

So, anyone else have any ideas for how to tackle this question?
